To make a donation, please print and complete the following form and return it and your donation to our office at:

In Memorial Donations
To make a donation in memory of a loved one, simply print and complete the attached donation form. Memorial donations can also be made at our local funeral homes. Acknowledgement cards will be sent to the family in honour of your gift.
